    Allergy not= to Side Effects.

    Much more important.

    Giving someone a drug they've had side effects to is one thing. Giving someone a drug that they've had an allergic reaction to is a major mistake.

    Allergy is more important than Fear, add it there.

    If a drug was stopped for Fear, "just type it in" :)

    I get far more people declined to get their meds filled on account of fear of side effects that developing an allergy!

    I'm not sure I know of an EMR that doesn't have a section dedicated to allergies.

    Certainly doctors would be used to having an allergy section.

    At least being able to easily add Allergy as the first word in the Reason column would be a good idea.

    Another problem would be the co-mingling of allergies and discontinued drugs makes reporting allergies to other physicians difficult. Does a doctor receiving my referral note really want to know the 4 NSAIDs, 5 SSRIs, 3 Sleeping pills and 2 statins the patient eventually stopped ?

    I think most doctor expect a section dedicated to allergies. Drugs stopped b/c of intolerances and the million other reasons could go in another category.

    OK keep fear, but just add Allergy.

    IDEA: add the word ALLERGY to the Instructions - and order the Old Meds with Allergies first.

    Another IDEA: each medication has a column (field?) in the database called Allergy - it is set to "N" for No. But when either method of adding an allergy is used ... it is Updated to Y. The Allergies and Old medications could be sorted on this (undisplayed) field ... with all the drug allergies listed first.

    bump.

    In primary care we deal with alot of discontinued medications.

    ALOT !

    To comingle discontinued drugs with allergies is dangerous.



    In reality, there are 3 sections needed.

    (1) Allergies

    (2) Drugs the patient doesn't want again.

    (3) Discontinued drugs

    If there were two sections I would merge (1) and (2) but make note in a notes section if it is an allergy or not.

    Here was a few mockup suggestions I had for the allergies.

    For myself: all allergies, I put the word ALLERGY in the instruction line.

    I'm in agreement that Allergies should have it's own tab, and (hopefully) it's own variable, like $allergies.

    Here's an example of the output into a consult note when I clicked "Previous Meds/Allergies" on a test patient:


    ALLERGIES AND FAILED MEDICATIONS:
    AMOXICILLIN 500MG 1 tab po TID
    PHENERGAN 25MG 1 tab po q 6 hrs prn "nausea"
    Penicillin Any Dose Allergic at any dos

    NOTE: To add penicillin as an allergy, you have to fill out all the fields, so I made it Med=Penicillin, dose=Any Dose, instructions=Allergic at any dose, admittedly cumbersome.


    For most patients we deal with this is a bit weird. Most patients just have some vague recollection they have been told they are allergic to penicillin as a child. Was it anaphylactic? Just a rash? Often not sure how severe, maybe they can answer that one at least. On the other hand, may have had a severe reaction to something and are very definite about it. So I think we should have something more akin to the synapse diagnosis dialog box -- 1. How was allergy established 2. By whom? 3. How severe? 4. When did it occur? This would be too detailed most of the time, but there are those times when it would not, and be very important to patient care.

    I probably also need a section that deals with "One Time Meds".

    I prescribe alot of "One Time Meds".

    I just dispense them as current meds and delete them when I next see them.

    Maybe Allergies ... should be a different colour ?

    We should, where possible keep colour consistent with function.

    Green - is usually action-oriented (Edit, Save).

    Red - is usually action-oriend (destructive nature) (Close, Cancel).

    How about using a Tab colour like ...Orange ..

    Exactly so the green shows that the button has been pushed to produce that list of items.
