Maybe a default country code could be implemented. All of my patients live in the same country. If space is a premium, having a default gender should be removed. We do have patients complain about having the wrong gender assigned from time to time.
Don't bother about the country code .. it's not used at present. There's a bug with the gender .. it is supposed to get that from the title but doesn't always,
