New York Workers Compensation requires the C-4 form, instead of the CMS-1500. Softwares like Medisoft and Eclipse allow you to plug in a custom module to generate the C-4 form.
Any doctor who's been in practice long enough will eventually get workers compensation cases. Ortho, PM&R, Neuro, Pain, and Physical Therapy, get workers compensation cases *all* the time. For that reason, softwares like Medisoft and Eclipse allow a plug-in module to print the C-4 form. Some of the better softwares have the C-4 built-in. If a NY doctor were to use Synapse, then he would need a second billing software just for C-4. In effect, Synapse forces NY doctors to have two billing softwares, which is ridiculous. Unless Synapse comes up with an *official* solution (and I stress the word "official"), Synapse will never be viable in NY.
Medisoft is a practice management software .... Synapse is an EMR. Synapse does not do billing. You will need a billing program as well.
Software like Eclipse ? I don't know any EMRs called Eclipse. I know of Eclipse the Development Platform. http://www.eclipse.org/
The Sections I have filled out in Red are certainly doable from Synapse. This is populating a form with Demographics. Is that what you were thinking of ?
That’s a strange question. Perhaps you misunderstood from the beginning. Yes, the entire thing. Every box and every field must be completely filled out, or your claims and bills will be denied by Workers Comp. The patient’s name, DOB, date of injury, how injured?, still working?, CPT, ICD, tax ID, charge amount, etc. The user needs to somehow enter the data, such as place of injury, into the database, and then when you print the C-4, such information is correctly placed on the C-4. Basically, instead of using CMS-1500, you’re using C-4.

Nobody really uses Eclipse as an EMR, but it’s monstrously popular as a billing software. Why so popular in NY? For the mere fact that it can do C-4 *easily and conveniently*.
The pdf form at http://www.wcb.state.ny.us/content/main/forms/c4.pdf is a pdf AcroForm. If you fill it out with Acrobat Reader, the font is proportional. If you type in 2 characters in the text entry box the font is huge, more letters makes the font smaller. This makes the form look a bit weird when filled out online. Do you know it the form has to be filled out in any particular way to get paid? Is the form at http://www.wcb.state.ny.us/content/main/forms/c4.pdf acceptable to payors or billing clearing houses? The CMS1500 form has to be filled out the way Medicare or your particular medical billing clearing house likes it.
Synapse doesn't do financial/practice management stuff at all. It does have an interface to a Practice Management Software called Zybex. at http://zybex.com . Synapse has a form design tool for overlaying data on pdf's.
The C-4 is not “machine read”. A person manually processes the form, so you can actually fill it out by hand, if you have legible penmanship.

Check out Eclipse. www.galactek.com It does both medical records and billing. Eclipse is an enormously popular billing software in NY, for the simple fact that it does C-4 so easily and effortlessly. However, Eclipse is very expensive.
