Result Types

    New Result Types I see....

    The Diabetic Foot Exam will not end up in my inbox really. I guess if a specialist did it, I could read that in the consult, but I'll still have to call that result a "Consult" and not a "Diabetic Foot Exam".

    The Diabetic Eye exam will *sometimes* end up in my box. I can ask the patient if they have gone but more often than not the optometrist will not send their report. Or they will, but it costs the patient $40, so I really never see them.

    There will have to be other ways to click these items as satisfied besides theresults inbox.

    Graham & Jason:

    I would add EGD (Upper Endoscopy); Social Work; ECHO; ECG; Record Requests;OTHER ( space to add your own).

    A "Personal" item to include Disable parking permits, Family leave of absence, etc. The description would be entered in the text area.

    Adding "Select Result Type" as the label tells the operator what to do.

    You use the same window for entering text and numerical results. Is it possible to fade or remove the Numeric Results area when entering text/image results?

    I tried to move over my whole list of "suggestions" from another thread but it seems that CS's split thread doesnt work !

    DrO .. you do know you can type whatever you want in that box eh ?

    With so many different practitioners receiving different things, being able to type it in is handy.

    For outpatient primary care, a list that covers the entire screen would be handy !

    DrO, any result can be typed into the field .. but there needs to be a limit on the selection, as too many makes it hard to pick out the values.

    I think it might be able to leave the numeric results area enabled only for the lab results, and disabled for everything else.

    Entering other reports/requests in the text result area helps you track non medical information pertaining to the overall management of the patient. These are usually part of the patient's chart and include equipment requests (beds, glucometers, CPAP, etc), Nursing home, ALF's, Hospice info requests, "medication management advice", formulary change requests etc.

    I think the add Text Results would benefit from it's own Popup Window. Of course I say that for most things. :)

    So far I want to manually add ... PSA for Prostate CA patients, Hb for selected patients, ESR for PMR patients.

    And a few other things.

    My Online (Web-based) Lab won't accept FireFox - it's IE only. I'd love to have Synapse Open up my patient's online labs ... maybe one day.

    DrO - do you have access to HL7 labs or a Web-based lab Interface ?

    I want a popup because .. i usually find a lab I want to add while in the Inbox. The current inbox is a popup from clicking [Add New] is it possible to assign it to a Function key as well ?

    (not that we haven't run out of function keys !).

    I tried to enter numerical results and realized that I need to remember the units. No time to enter all of that. In Type there should be a drop down menu with the most common results: Hemoglobin, Glucose, Tot-Chol, LDL,HDL, Trigs, PSA, PT, INR, PSA, EF(ejection fraction), Microalbumin/Creat ratio, Other, etc.

    Associated with each and blind to the user the LOINC# ( not widespread in the US), and the units. We should be able to select in Settings, US and TROTW[6]( the rest of the world, i.e., the metric system. As far as I am concerned there is no need for Source. Thus, the user would select the Type and enter the Value. Easy!!

    Not pretty that's for sure. But you could have a whole row of them ... and they could be grouped somewhat. Ideally there could be a multi-column dropdown from just one location.

    If I have edit-lists for the common tests, it can put up the default units automatically.

    Let me know what the common units are for those tests.

    I have source there as I get results from 4 different labs.

    DrO, who's your lab source? LabCorp ?

    My experience is that $$$ is only charged by EMRs, and lab software, not by labs themselves. Ask them if you can get your labs sent to you by encrypted email, sftp, or other electronic method. You may be surprised.

    Thinking about this again .. I very much doubt that most docs want to add extensive results this way. Most will just scan the results and leave it at that. You might do it a couple of times, but it will get tired really quickly.

    I just checked .. it already does that. If you enter in some text in the Result Type, you do have to hit enter to activate this feature. Otherwise the drop down list will do it for you.

    Loinc is a US invention, and is being adopted by many .. if not all .. EMRs.

    It is the way in which HMGs are satisfied.

    The LOINC code is automatically entered if the system has been trained.. which is why the source should be stated.

    As an analyst that has worked with many HL7 lab interfaces, most labs I've seen absorb the cost of interfaces as part of their service.

    What lab do you use?

    I get results from Labcorp (they set up a printer in my office), Quest, a local lab and a state lab based in So. FL. The latter is amenable to provide digital results.

    Dr. O,

    I'm interested to hear what Quest and Labcorp are wanting to charge you for a results interface. Its possible that your practice does not have enough volume with these labs to provide you a free interface.

    Alternatively, I believe they may have a website where you can manually download HL7 results.

    Also, the Synapse chickenscripts web services functionality may be of use here to allow you to log in automatically to their websites and let you view them there.
